Relevant history and current clinical information allow the consultant to assess suitability and determine the recommended next step.

Use the following headings and include only documents relevant to the referral question.

    Describe the presenting concern, duration and severity, effect on functioning, and the specialist opinion or treatment being requested.

  2. 02

    Young person and family details

    Include identifying and contact details, parent or guardian information, and confirmation that the family is aware of the referral.

  3. 03

    Relevant background

    Summarise developmental, medical, educational, family and psychiatric history, current medication, allergies and professionals already involved.

  4. 04

    Risk and safeguarding

    Describe current or previous risk, protective factors, safeguarding concerns and action already taken.

  5. 05

    Supporting information

    Attach relevant clinical correspondence, previous assessments, school reports or other requested documents using the approved secure route.

The consultant considers the referral and may ask the referrer to clarify the clinical question or provide additional relevant records.
Accepted referral
If accepted, the family receives the proposed assessment format, estimated waiting time, preparation requirements and fees.
Professional communication
With consent, relevant findings and recommendations are communicated to the referrer.
Treatment arrangements
Any prescribing, monitoring or shared‑care arrangements are agreed individually with the family and relevant clinicians.
